Medicare Diabetes Services Order Form (PDF, 100K)
The American Association of Diabetes Educators (AADE) and the
American Dietetic Association (ADA), in cooperation with the
American Health Quality Association (AHQA), have developed a
Diabetes Services Order Form. This form is designed as a simple,
convenient way for a physician to refer Medicare patients with
diabetes to a Medicare-certified diabetes educator for diabetes
self-management training (DSMT) and to a registered dietician for
medical nutrition therapy (MNT). The form includes key information
required to meet Medicare regulatory requirements for MNT and/or
DSMT referrals.

The Agency for Healthcare Research (AHRQ) has developed
Diabetes Care Quality Improvement: A Resource Guide for State
Action and its companion workbook, Diabetes Care Quality
Improvement: A Workbook for State Action. These materials were
designed in partnership with the Council of State Governments to
help States assess the quality of diabetes care and create quality
improvement strategies.
