At the end of the presentation, the participant will be able to:
Describe the critical staffing components needed to form a successful team to implement Diabetes
Self Management Support utilizing the Chronic Care Model.
Identify components needed to cultivate buy in from administration, providers, and staff.
List three aspects of the planning, implementation, and evaluation of the Diabetes Self Management Support utilizing the Chronic Care Model.
Describe the barriers to access to healthcare and determinants of health disparities
List aspects of how to improve access to affordable healthcare, provide health professional training, promote health promotion and disease prevention and promote health professional recruitment and retention.
Describe how to improve the quality of medical care by providing medical providers with standard for preventive health care practices.
